A fatal midair crash on Wednesday between a Black Hawk helicopter and an American Airlines passenger jet has highlighted several key issues. Air traffic control at Reagan National Airport was handled by a single operator, instead of the usual two, due to early staff departures and shift changes. Additionally, the Black Hawk may have deviated from its approved flight path, flying at an altitude higher than permitted. The National Transportation Safety Board (NTSB) is leading the investigation into the crash, which resulted in 67 fatalities.
